How this plan works

Five vendors, one broker.

A self-funded plan usually involves stop-loss insurance, which caps your cost when claims get very large; a TPA (third-party administrator) that processes claims; a provider network; a PBM (pharmacy benefit manager) that runs prescription coverage; and ancillary coverage like dental and vision. Each is a separate contract, invoice, and renewal.
